SERVICE CONTRACT

TERMS OF LEASE 

Patrick Miller Roll-Off Services will provide dumpster disposal service using our roll-off containers. Service will be provided on the day requested when using our online ordering software. Extending rentals will incur additional fees. Please be advised the latest delivery time is 5:00 pm on the scheduled delivery date and the earliest pick up is 7:00 am on the scheduled pick up date. Unless there is a time specified by the customer, delivery and pick-up routes are scheduled in the office based on location. 

Additional Charges: 

All customers are responsible for the total weight of the contents of their dumpster(s). All customers are responsible for scheduling the removal of their dumpster(s) or dumpster(s) will be picked up on the 7th day. All customers are responsible for ensuring their dumpster(s) are not overloaded. Customers shall inspect the dumpster upon delivery for any existing damage. Upon removal of the dumpster, Patrick Miller Roll-Off Services shall be entitled to charge the customer for the repair or replacement costs attributable to any damage to the dumpster while in the customer’s possession. The customer shall be liable for any repair or replacement costs. Upon removal, the customer authorizes Patrick Miller Roll-Off Services to collect any additional disposal, repair, or replacement costs attributable to the customer. Please view the dumpster description for items that incur additional fees such as but not limited to mattresses/boxsprings, tires, appliances, electronics, and furniture. 

Exceeding the stated weight allowance will result in additional charges of the below-stated amounts

C&D Box 

10 yard – 4000 lbs included (2 tons) ($434.00 + $6.00(tax) = $440.00)

30 yard – 12000 lbs included (6 tons)($615.00 + $18.00(tax) = $633.00)

Please note if you state the box to be C&D and upon disposal, it is discovered to be MSW the correct taxes and fees will be applied at final billing. 

Any debris above the max fill line may result in refusal of service and off-loading may be required. A dry run charge can range from $95/Ton to up $175/Ton per occurrence based on delivery/pick-up location. Blocking or impeding retrieval of a roll-off on the agreed collection date will result in a dry run charge. 

Cleaning due to the loading of restricted or hazardous materials may be billed at a 3rd party rate plus a markup. 

Waste Material 

Non-Hazardous Solid Waste Only. Customer agrees not to put any waste that is liquid, or any waste that is, or contains, radioactive, volatile, corrosive, highly flammable, explosive, biomedical, biohazardous, infectious, toxic, and/or any hazardous wastes or substances (“Prohibited Waste”) into roll-off containers. Prohibited Waste includes, but is not limited to, paint/stain, batteries, ashes, oil/gasoline, vehicle parts, sewage sludge, asbestos, etc. Any hazardous waste in the dumpster will result in a $200.00 minimum fee plus all relatable disposal fees. 

Burn Loads 

For any burn loads or loads that will have burned debris, a fire report is required before the load can be dumped. It is the homeowner’s/contractor’s responsibility to obtain this from the Fire Department that responded to the fire. A burn load can not be dumped for 14 days after the incident occurred. There will be a $65 burn load fee assessed once dumped. IF the burn load contains a home/mobile home built in 1990 and older you will need to obtain an Asbestos Survey before the load can be dumped. 

Inspection/Rejection of Prohibited Waste 

Title to and liability for Prohibited Waste shall remain with Customer at all times. Patrick Miller Roll Off Services shall have the right to inspect, analyze, and/or test any waste delivered by the customer. 

Acknowledgment 

Customer acknowledges that Patrick Miller Roll-Off Services shall not be liable for any damage to driving surfaces resulting from Patrick Miller Roll-Off Services trucks serving containers on the agreed-upon areas and the surroundings. Customer acknowledges that they are not allowed to move around any roll-offs with their personal equipment or a third party’s equipment. 

Payment Terms 

Prepayment at the time of reservation is required by debit card or credit card, Patrick Miller Roll-Off Services will keep that card on file until the account is at a zero balance. 

By signing, I am acknowledging that I have read and agree to the terms listed in the entirety of the contract.
